Patients are admitted into our services by referral from the patient's physician or clinical case manager. The admission process includes a comprehensive physical assessment and treatments ordered by the physician. The homecare team will also ensure that your medication regimen is evaluated and monitored to provide maximum benefit and safety.

HomePoint Home Care Admitting Criteria
The following list provides guidance on allowable conditions where homecare services may be helpful to patients and/or caregivers:
  • Frequent hospital admissions, physician office visits or after-hours phone consultations
  • Medication management issues
  • New diagnosis or disease
  • Re-occurrence or uncontrolled symptoms related to diagnoses such as COPD, CHF, diabetes, pneumonia, wounds, ulcers, infections or other chronic and long-term illnesses
  • Post-surgical monitoring
  • Administration of IV's or injectable medications
  • Education related to disease process: diet, glucose monitoring, infection control
  • Complications with healing process
  • New home oxygen
  • Indwelling catheter
  • G-Tube care and use
