Northwest Home Health understands that the best place for you to recuperate is in the comfort of your own home. We provide a full range of home care services, including:

Skilled Nursing Services

Northwest Home Health offers skilled nursing services to provide assessment, evaluation, teaching, and medication management of your pertinent diagnoses.  Nursing services may also include:  IV administration, wound care, and ostomy care.

Therapy Services

Northwest Home Health offers physical therapy services to improve functional mobility.

Home Health Aides

Patients are admitted into our services by referral from the patient's physician or clinical case manager. The admission process includes a comprehensive physical assessment and treatments ordered by the physician. The home care team will also ensure that your medication regimen is evaluated and monitored to provide maximum benefit and safety.

Northwest Home Health Admitting Criteria

The following criteria is required for home health admission:

  • A physician order
  • The need for intermittent, skilled services (nursing or therapy)
  • Most major insurances require you to be home bound
The following list provides guidance on allowable conditions where home care services may be helpful to patients and/or caregivers:
  • Frequent hospital admissions, physician office visits or after-hours phone consultations
  • Re-occurrence or uncontrolled symptoms related to diagnoses such as COPD, CHF, diabetes, pneumonia, wounds, ulcers, infections or other chronic   and long-term illnesses
  • Medication management issues
  • New diagnosis or disease
  • Post-surgical monitoring
  • Administration of IV's or injectable medications
  • Education related to disease process: diet, glucose monitoring, infection control
  • Complications with healing process
  • New home oxygen
  • Indwelling catheter
  • G-Tube care and use
